Troponin

A heart muscle protein released into the blood when heart cells are stressed or injured.

What it measures

Troponins are proteins that heart muscle cells use to contract, and they are normally kept inside those cells. The test measures the small amount of troponin I or troponin T that has leaked into the bloodstream. Modern high-sensitivity versions can detect tiny traces.

Why a doctor orders it

Mainly ordered in urgent settings when chest pain, breathlessness, or other symptoms raise the question of a heart attack. It is usually repeated over several hours, because the pattern of change matters as much as any single value.

What is generally associated with a higher result

Broadly associated with stress or injury to heart muscle from many causes, of which a heart attack is only one. Kidney disease, fast heart rhythms, heart failure, severe infection, and very strenuous exercise are all recognized non-heart-attack causes. High-sensitivity assays pick up small elevations often, so results are read together with symptoms, ECG findings, and the trend over time.

What is generally associated with a lower result

Low or undetectable values are the expected finding in most people. Low values are generally not pursued.

Everyday things that move this number

Not every shift in a value reflects disease. These are ordinary factors known to affect this particular measurement:

  • Very strenuous recent exercise, such as a long race
  • Reduced kidney function, which slows how fast troponin is cleared
  • Biotin supplements, which can interfere with some lab methods
  • Age and sex, which shift what counts as a typical level
  • Which assay the lab uses - high-sensitivity and older versions read on different scales

  • Was this a high-sensitivity troponin, and was the level rising, falling, or stable across draws?
